Bookkeeping & Payroll

Keep the financial records current and ready for authorized review.

We map how money records move through your practice, fix the handoffs, configure the controls, and build the automations or help hire the person you need. Then we test the setup, train your team, and hand it over.

Situation

Is this your problem?

Sound familiar?

  • Bank deposits do not tie back to the practice management system.
  • Transactions wait weeks for categories, receipts, or an owner.
  • Vendor bills live across inboxes, portals, and paper stacks.
  • Payroll inputs arrive late or change after the cutoff.
  • Month-end reports explain last month after the next one has started.
  • Your CPA spends paid time reconstructing records instead of reviewing them.
Scope

What this workstation owns.

From a completed transaction to a clean record, an approved payment, or a documented exception.

Daily cash and deposit control
  • Match practice-management deposits to processor batches and bank activity
  • Reconcile card, check, cash, and financing deposits by location and date
  • Record processor fees, refunds, chargebacks, and timing differences
  • Flag missing, duplicate, split, or short deposits for review
  • Keep a daily exception list with an owner and next step
Transaction records and reconciliation
  • Categorize bank and card transactions under the approved chart of accounts
  • Collect receipts, statements, and supporting documents
  • Reconcile bank, card, loan, and clearing accounts
  • Maintain vendor records and consistent category rules
  • Route uncertain or unusual transactions to the bookkeeper, manager, or CPA
Accounts payable and purchasing records
  • Capture vendor bills, due dates, terms, and supporting documents
  • Check bills for duplicates, unexpected changes, and required approvals
  • Prepare payment batches for named human review
  • Record approved payments and keep the vendor ledger current
  • Follow up on credits, disputed charges, and missing statements
Payroll coordination
  • Gather approved hours, time-off, bonuses, commissions, and deductions
  • Check payroll inputs against schedules and approved compensation records
  • Prepare the payroll run and exception list before the cutoff
  • Coordinate filing notices and payroll-tax questions with the payroll provider and CPA
  • Record the approved payroll journal and reconcile payroll clearing accounts
Month-end close and reporting
  • Complete the close checklist and clear open reconciliation items
  • Prepare production-versus-collections and deposit summaries from source systems
  • Update cash-flow, payable, payroll, and expense views
  • Assemble a review packet for the owner, bookkeeper, and CPA
  • Lock the period only after named human review and document later changes

Patient balances, insurance claim payments, EOB posting, refunds, and write-offs belong to Claims & Billing until they reach the accounting record. Campaign performance and marketing-spend analysis belong to Marketing & Acquisition. Tax advice, tax positions, filing decisions, and attest work stay with the practice's CPA or other licensed professional. This workstation prepares records and exceptions. It never releases a vendor payment, submits payroll, changes compensation, or makes a tax decision without named human approval.

Metrics

How do we measure success?

Five measures. Targets agreed from your practice's baseline.

Unreconciled deposits
Daily deposits in the practice system that have not been matched to processor and bank records by the agreed cutoff.
Overdue payable exceptions
Vendor bills past their due date with no approved payment, documented dispute, or named next step.
Payroll input corrections
Payroll entries changed after the first review, tracked per pay cycle and by cause.
Close completion time
Business days from month-end until reconciliations, review schedules, and the CPA packet are complete.
Uncategorized transactions
Transactions with no approved category or supporting record at the agreed review date.
Automation

AI agents and automations for this workstation.

A few examples. We build around your practice's controls and existing systems, not a fixed menu. Every payment, payroll submission, compensation change, journal approval, and filing decision stays with a named person.

The manual way

  • Export deposit reports from the PMS.
  • Download processor settlement files.
  • Compare bank activity one line at a time.
  • Investigate timing and fee differences.
  • Build a spreadsheet of unresolved items.

With AI agents & automation

  • Scheduled exports land in one controlled workspace.
  • Fixed rules match deposits by amount, date, batch, and location.
  • Fees and expected timing differences are separated.
  • Only unmatched or low-confidence items reach a person.

Systems needed

  • Practice management system
  • Payment-processor settlement data
  • Read-only bank feed

Human control

A named bookkeeper or manager reviews exceptions and approves any accounting entry. Refunds, transfers, write-offs, and bank actions stay outside the workflow.

Full build specification
Trigger / problem
A daily deposit window closes, but the PMS total, processor settlements, and bank activity have not been tied together.
What it reads
Approved read-only fields from the PMS deposit report, processor batch file, and bank feed: date, location, payment type, gross amount, fee, net amount, batch ID, and posting status.
What it decides
Whether records meet the practice's fixed matching rules, whether a known timing or fee rule applies, and whether the item needs review. It does not decide that a real difference is acceptable.
What it does
Creates candidate matches, separates documented processor fees and timing items, and sends unresolved differences to an exception queue with source links.
Human approval / takeover
A named bookkeeper or manager reviews exceptions and approves any accounting entry. Refunds, transfers, write-offs, and bank actions stay outside the workflow.
Systems needed
PMS reporting access, processor exports or API access, read-only bank data, and the accounting system. We verify export quality and identifiers before scoping.
Privacy / access
Uses the minimum fields needed for matching. Patient names and clinical details are excluded when transaction IDs and totals are enough. Bank access is read-only.
Logging & failure handling
Every imported file, proposed match, override, and exception is logged. Missing files, duplicate batches, stale feeds, or out-of-balance totals stop the affected run and alert the owner.
Success metric
Share of daily deposits matched by the cutoff, number and value of unresolved differences, and age of the oldest exception.
What the practice owns at handoff
The matching rules, connectors, exception queue, access map, closeout checklist, test cases, and operating guide inside practice-controlled accounts.
Good fit when
The same deposit reports are compared every day and most differences follow known timing, batch, or fee rules.
Not a fit when
Source exports lack stable identifiers, bank access cannot be restricted, or the practice expects the workflow to approve losses or move money.

The manual way

  • Watch several inboxes and vendor portals.
  • Type each bill into the accounting system.
  • Search for prior invoices and purchase records.
  • Chase approvers by message.
  • Rebuild the payment list before every run.

With AI agents & automation

  • Bills enter one controlled intake queue.
  • Key fields and supporting documents are extracted.
  • Duplicate, amount, vendor, and approval checks run first.
  • A complete approval packet reaches the named approver.

Systems needed

  • Vendor-bill inbox or portal
  • Accounting system
  • Approval matrix

Human control

A named reviewer confirms vendor, amount, category, documentation, and due date. A separately authorized person releases payment. New vendors, bank-detail changes, rush requests, and conflicts always stop for review.

Full build specification
Trigger / problem
A bill arrives by approved email, upload, or vendor portal and needs to become a reviewed payable without getting lost or paid twice.
What it reads
The invoice image or PDF, vendor master, prior bills, approved purchase record when one exists, coding rules, approval matrix, due date, and payment status.
What it decides
Which known vendor record and draft category fit, whether the bill appears duplicate, which approver the matrix names, and whether the data is complete enough to route. Low confidence is an exception.
What it does
Creates a draft bill or intake record, attaches the source document, flags possible duplicates or changed payment details, and routes the packet for review.
Human approval / takeover
A named reviewer confirms vendor, amount, category, documentation, and due date. A separately authorized person releases payment. New vendors, bank-detail changes, rush requests, and conflicts always stop for review.
Systems needed
A controlled bill-intake channel, accounting platform, vendor master, document store, and approval path. Optional purchase-order or inventory data is used only where the practice already maintains it.
Privacy / access
Financial access is role-based. Vendor tax forms and bank details are restricted. The workflow never sends sensitive records to an unapproved model or destination.
Logging & failure handling
Source, extraction, edits, duplicate checks, approvals, and status changes are logged. Unreadable files, conflicting invoice numbers, or changed remittance instructions block routing and alert a person.
Success metric
Time from receipt to reviewed payable, duplicate rate caught before approval, bills with complete support, and overdue exceptions.
What the practice owns at handoff
The intake address, extraction schema, vendor rules, approval matrix, exception types, audit log, and operating guide.
Good fit when
Bill volume is repetitive, invoices are machine-readable, and approval authority is already documented.
Not a fit when
Approvers are undefined, vendor records are unreliable, or the practice wants software to decide whether an expense should be incurred.
Payroll-input agentgather → check → queue

Built forThe payroll provider remains the system of record and recurring inputs can be checked against approved sources.

The manual way

  • Collect changes from messages and spreadsheets.
  • Compare hours against schedules by hand.
  • Re-enter bonuses, deductions, and time off.
  • Ask managers about missing approvals.
  • Discover errors after payroll is submitted.

With AI agents & automation

  • Approved inputs follow one cutoff and format.
  • Fixed checks compare records and surface conflicts.
  • Missing approvals and changed pay details stop the packet.
  • A named payroll approver receives one exception summary.

Systems needed

  • Timekeeping and schedule data
  • Payroll platform
  • Approved compensation records

Human control

The authorized payroll approver resolves every exception, reviews the provider's final register, and submits payroll. The workflow has no ability to submit payroll or change bank details.

Full build specification
Trigger / problem
A payroll cutoff approaches and the practice needs one checked input packet before an authorized person runs payroll.
What it reads
Approved time records, schedules, time-off records, standing compensation data, authorized bonus or commission inputs, deductions, prior payroll, and cutoff rules.
What it decides
Whether required inputs are present, whether fixed comparisons pass, and which conflicts need review. It never decides pay rates, eligibility, classification, withholding, or tax treatment.
What it does
Builds a draft payroll-input packet, lists changes from the prior cycle, and routes missing approvals or conflicting values to the named owner.
Human approval / takeover
The authorized payroll approver resolves every exception, reviews the provider's final register, and submits payroll. The workflow has no ability to submit payroll or change bank details.
Systems needed
Timekeeping, scheduling, payroll, secure document storage, and an approved compensation source. We confirm which system is authoritative for each field.
Privacy / access
Payroll and employee data use least-privilege access, restricted storage, and named roles. Social Security numbers and bank details are not copied into general workflow tools.
Logging & failure handling
Imports, checks, changes, approvals, and file versions are logged. A missing source, changed account, late input, or failed sync blocks completion and alerts the payroll approver.
Success metric
Corrections after first review, late inputs, unresolved exceptions at cutoff, and pay cycles submitted only after documented approval.
What the practice owns at handoff
The cutoff calendar, input form, check rules, approval map, exception report, access design, test payroll, and operating guide.
Good fit when
The payroll provider remains the system of record and recurring inputs can be checked against approved sources.
Not a fit when
Compensation policies are unsettled, employee classifications need legal advice, or the practice expects an agent to submit payroll without review.

The manual way

  • Rebuild the close checklist each month.
  • Export reports from several systems.
  • Copy totals into a spreadsheet.
  • Chase unexplained differences and missing support.
  • Send the CPA an incomplete folder and answer follow-ups later.

With AI agents & automation

  • The same close checklist opens every period.
  • Source reports and reconciliations are assembled in order.
  • Missing support, unusual changes, and stale exceptions are flagged.
  • A draft owner and CPA packet is ready for named review.

Systems needed

  • Accounting system
  • PMS and payroll reports
  • Controlled document store

Human control

Named reviewers approve entries, accounting treatment, tax treatment, management explanations, period lock, and every external handoff. Unusual vendor or bank changes go to an independent verifier.

Full build specification
Trigger / problem
The period ends and the practice needs a complete, traceable close packet without rebuilding the process from memory.
What it reads
The close checklist, reconciliations, trial balance, prior-period reports, PMS production and collections reports, payroll summaries, payable aging, fixed review thresholds, and open exceptions.
What it decides
Which required schedules are missing, whether totals agree under fixed rules, which movements cross review thresholds, and which item owner should receive an exception. It does not decide accounting treatment.
What it does
Tracks checklist status, assembles linked source schedules, drafts variance notes that cite the underlying values, and packages unresolved questions for the bookkeeper, owner, or CPA.
Human approval / takeover
Named reviewers approve entries, accounting treatment, tax treatment, management explanations, period lock, and every external handoff. Unusual vendor or bank changes go to an independent verifier.
Systems needed
Accounting platform, read-only reporting access to PMS and payroll, close checklist, document store, and secure CPA handoff. A business-intelligence layer is optional, not required.
Privacy / access
Reports are restricted by role and shared only through approved destinations. Employee-level payroll data and patient-level records are excluded from owner summaries unless specifically required and authorized.
Logging & failure handling
Every source version, checklist completion, proposed note, reviewer change, and approval is logged. Out-of-balance records, stale data, or missing support keep the affected section open rather than creating a false clean close.
Success metric
Business days to reviewed close, open exceptions at close, post-close corrections, and CPA follow-up caused by missing records.
What the practice owns at handoff
The close calendar, checklist, report definitions, thresholds, connectors, packet template, access map, test close, and operating guide.
Good fit when
The practice has named source systems and wants a repeatable evidence trail around review and handoff.
Not a fit when
Books require reconstruction, accounting policies are undecided, or the practice needs tax, audit, or attest work from a licensed professional.
